SCHEDULE

ON-CALL RESPONSIBILITIES
On-call ultrasounds and FB removals are the most common situations. The on-call schedule is to be coordinated between the service. A bonus applies to every on call.

Referral letters, phone communication with clients and veterinarians, training of interns, attend DVM and all staff meetings, attend local association meeting and holiday gatherings.

Staff Internist
Location: Westford, MA
Primary Responsibilities
Responsibilities are primarily patient care and the communication of the care provided. Our staff internists examine, perform diagnostic testing, and treat patients within their prevue of internal medicine. The majority of caseload is scheduled and referred from local general practices with some intrahospital referrals. Communication of pertinent information with referring veterinarians and clients is performed though in-person consultations, telephone, email, and occasionally text-messaging.
Care and Diagnostic testing may include clinical pathology, radiographs, abdominal ultrasound, non-cardiac thoracic ultrasound, aspirates, or biopsies as well as procedures performed under general anesthesia such as endoscopy, rhinoscopy, CT or MRI Scan. Anesthesia protocols are dictated by the internist with assistance from other services/colleagues if desired or indicated. Anesthesia induction, monitoring, and recovery is performed by trained dedicated technicians with direct or indirect supervision from the staff internist.
Other than performing abdominal ultrasounds and consultations for the emergency service, no direct management of emergency or intensive care cases is expected. No on-call or holiday responsibilities are expected. If desired, voluntary services may include after-hours consultations or ultrasounds for the emergency service and would be compensated.
All hospitalized patients are cared for by our team of criticalists and emergency clinicians. No inpatient responsibilities or case management is required unless desired.
Desired Skillset
Our Staff Internist should be proficient in all aspects of the diagnosis and treatment of disease in the prevue of internal medicine. Additional skills include comfort with flexible and rigid endoscopy. Proficiency or the desire to become proficient in abdominal ultrasound and non-cardiac thoracic ultrasound is desired.

Each doctor is assigned a dedicated technician and a dedicated technician assistant. In addition, floating technicians and technician assistants are available to assist when needed.
Schedule
The staff internists are scheduled 4 days/week which include occasional Saturdays. Appointments are scheduled in the morning with the afternoons reserved for diagnostic procedures. Time to perform ultrasounds for the emergency service are built into the daily schedule. New-client appointments are limited on Saturdays which are typically reserved for recheck appointments and ultrasounds for the emergency service.
On Call Responsibilities
None unless desired. On-call is voluntary with additional compensation.

RDVM Protocol
Referring veterinarians are strongly encouraged to submit a referral made available on our website. Often a referring veterinarian will have the pet owner contact the internal medicine service resulting in a reflexive call to the referring veterinarian for medical records and history. Some referring veterinarians prefer to speak with a dedicated technician or the staff internist directly by phone. Following a consultation, staff internists draft a referral report which is sent to the referring veterinarian typically within 24 hours of the visit. Copies of diagnostic test results received after the visit are sent to the referring veterinarian as results are reported. Our referring veterinarians have learned that we do not perform out-patient ultrasounds without a consultation. Sick patients referred for same-day ultrasounds and/or consultations that cannot be accommodated in the schedule are referred to our emergency service for evaluation and care. Our internists have very cordial relationships with our referring veterinarians and are always willing to discuss cases and provide advice when they are available.
Interns / Residents
Although we have hosted many veterinary student externs, we currently do not have an internship or residency program. Our desire is to start an emergency medicine focused internship and a rotating internship in 2021.
Administrative Responsibilities
No administrative responsibilities are expected. Team members who demonstrate a proclivity and desire for leadership are placed in administrate roles as the positions present.
Teaching Responsibilities
We expect all of our team members to share their knowledge and experience with their teammates. No formal or didactic program is in place although doctors and senior technicians will often voluntarily hold informal internal continuing education discussions with the team.
Community Outreach Desired
The hospital annually holds a continuing education event locally for referring veterinarians; participation as a presenter is greatly encouraged and appreciated but is ultimately voluntary.

The majority of caseload is scheduled and referred from one of the more than 130 local general practices that actively refer to our facility. Intrahospital referrals from other departments, especially emergency and neurology are common. Communication of pertinent information with referring veterinarians and clients is performed though in-person consultations, telephone, email, and occasionally text-messaging. Care and Diagnostic testing may include clinical pathology, radiographs, echocardiograms, electrocardiograms, Holter monitor interpretation, as well as procedures performed under sedation or general anesthesia such as thoracocentesis or pericardiocentesis. Anesthesia protocols, when needed, are dictated by the cardiologist with assistance from other services/colleagues if desired or indicated. Anesthesia induction, monitoring, and recovery is performed by trained dedicated technicians with direct or indirect supervision from the staff cardiologist. Other than performing echocardiograms and consultations for the emergency service, no direct management of emergency or intensive care cases is expected. No on-call or holiday responsibilities are expected. If desired, voluntary services may include after-hours consultations, echocardiograms, or ECG interpretations for the emergency service and would be compensated. All hospitalized patients are cared for by our team of criticalists and emergency clinicians. No inpatient responsibilities or case management is required unless desired.

3) The candidate should be motivated and excited to build a service to their liking that is centered on excellent patient care, client service, communication and team building.
3) While oncologic surgeries may be a large part of the caseload, other services such as orthopedic, neurologic or minimally invasive are welcomed.

The successful candidate will see appointments and manage cases 4 days a week. Initially 2 procedures a day increasing to 3 or 4 cases a day as needed.
The candidate will collaborate with other departments as needed. Dental patients often will have Cardiology and Internal medicine consults and ultrasounds as needed.
Zoo dentistry Police Canine Units also compromise a significant portion of the case load.
We are beginning a Stem Cell Stomatitis in Cats study in conjunction with UC at Davis and OVC
We lecture at Intern rounds 2 to 3 times during the year
In Non Covid times we give a mini lecture on Veterinary Appreciation Day for our referring veterinarians . All specialty groups do a lecture on that day. This is an annual event.
We respond to advice emails and calls as possible.

On Call Responsibilities
There is an on call fee billed to the client for both a technician and the veterinarian for each case. The ER veterinarians may ask for advice and will stabilize patients for us. The on call schedule is open for discussion and is very flexible.
Projected First-Year Caseload
I would expect in the vicinity of 500 -700 patients depending on how many cases the Candidate is comfortable doing.

The VEC pays Sharon French Veterinary Professional Corp all of the billings once a month. The VEC bills SFVPC the amount they are entitled to.
A base plus production beyond a threshold amount. The production % will be calculated on the amount remaining after the VEC has calculated their percentage of the total billings.
Straight production is also possible.
If the Candidate chooses to be an employee then CVO license and Liability Insurance will provided.
One conference annually will be provided for to a cost of up to $2000

The Dentistry Department is a close knit group of 3 technicians, 1 liasion and 2 ACAs. The technicians are gifted and supported by our two anesthesiologists we do patients of all risk levels.
We have 3 dental stations including 3 xray generators , Scanex and CR7 . We are looking at cone beam CT.
We have access to CT and MRI. The MRI is changing to a high field unit shortly.
We have an Antech lab across the hall including cytology to give rapid results.
The main dental suite has cages to keep our patients.
Each Doctor has their own private office.
The people in the dentistry department are gifted, loyal, supportive and some of the most fun people in the clinic. It is important to stop and smile and take a breath. The Dentistry team ensures that happens.
Describe the best aspects of being a Veterinarian at your hospital
As veterinary medicine advances – it is so important to be able to seek advice about problems that are not in our immediate field of expertise. With so many specialist departments and excellent ER and critical care departments( we have 2 criticalists – our second criticalist will be joining us in February) our patients get everything they may need.
We can go home at night knowing our patients will be cared for.
We truly do some amazing things at the VEC. We recently had Jerry who jumped in front of an attacking Pit Bull to protect his 4 lb house mate. Jerry, who is slightly bigger, should have died but the Jack Russell Terrier in him helped him survived. His neck was ripped open and his entire face was crushed , he had multiple fractured ribs. The collaborative effort for all departments was amazing. Jerry now 2 months post attack was in recently to have one of the appliances removed. He is amazing and a testament to what can be done if the spirit is there and we all work together.
